Working under the Director of Acute and Clinical Services, Health Navigators are essential in aiding both patients entering the Emergency Department and those with complex discharge needs. Your expertise will guide patient and family advocacy, addressing their medical, social, and spiritual requirements while facilitating access to community resources. Make a significant impact in the lives of vulnerable populations with your strong advocacy skills.
Key Responsibilities:
• Assess and address patient and family needs
• Provide advocacy and resource connection during care
• Lead discharge planning and family meetings
• Identify barriers to access and support services
• Collaborate with community resources and practitioners
Requirements:
• Relevant health degree with professional registration
• Valid Saskatchewan Drivers License
• CIPC and Vulnerable Sector Check
• Strong communication and crisis management abilities
• Knowledge of Indigenous practices and traditions
