Key Responsibilities:
• Assess patient and caregiver needs for personalized care
• Oversee coordination of hospice services and patient care plans
• Provide direct patient care, including symptom management
• Educate patients and families on disease progression
• Participate in on-call care after hours
Requirements:
• Graduate of a state-approved nursing program
• Current Illinois nursing license required
• One year of hospice experience preferred
• Certification in hospice and palliative care preferred
• Excellent communication and leadership skills
