The main purpose of this position

The Manager, Interdisciplinary Primary Care is a senior operational leadership role responsible for the oversight, performance, and continuous improvement of interdisciplinary primary care services.

The Manager ensures high-quality, client-directed, trauma-informed, and harm reduction–based care for individuals experiencing multiple barriers to health access, including homelessness, housing instability, mental health and substance use concerns, and justice involvement.

This role has a strong focus on Quality Improvement (QI), operational excellence, workforce development, and alignment with clinical priorities, organizational strategy, and Ontario Health Team expectations. The Manager has a direct impact on service quality, client outcomes, staff experience, and the successful integration of interdisciplinary primary care within the broader health system.

Responsibilities

Operational & Team Leadership

  • Provide operational leadership, oversight, and coordination of interdisciplinary primary care services.
  • Translate strategic and clinical direction from the Director into effective day-to-day operations.
  • Exercise decision-making authority related to staffing, service delivery, and operational priorities within established organizational and clinical frameworks.
  • Directly supervise assigned staff, including recruitment, onboarding, training, scheduling, performance management, coaching, and professional development.
  • Foster a collaborative, accountable, and psychologically safe team culture grounded in trauma-informed, harm reduction, anti-oppressive, and client-directed practice.
  • Ensure appropriate staffing models, role clarity, and workload distribution to support sustainable care delivery.
  • Coordinate and oversee the ordering and availability of supplies, equipment, and operational resources required to support interdisciplinary primary care services.
  • Lead regular team meetings, care coordination discussions, and case review processes.
  • Participate in Manager On-Call or escalation processes as required.

Quality Improvement & Practice Excellence

  • Lead and champion continuous QI across the IPCT using evidence-informed and data-driven approaches.
  • Ensure high-quality programs and services through a continuous quality improvement and evaluation approach.
  • Monitor service delivery, clinical workflows, and outcomes to identify gaps, risks, and opportunities for improvement.
  • Lead and implement QI initiatives aligned with organizational priorities, Ontario Health requirements, and best practices in primary care.
  • Oversee audits, incident reviews, and quality assurance activities, ensuring timely follow-up, accountability, and organizational learning.
  • Ensure standardized documentation practices, effective use of Electronic Health Records (EHR), and data integrity.
  • Collaborate with the Director on clinical policy development, practice guidelines, and care pathways.
  • Promote reflective practice, incorporation of lived and living experience, and continuous service refinement.
  • Supervise and evaluate students.

Clinical & Risk Oversight

  • Provide guidance and oversight to staff managing complex clinical and psychosocial situations, escalating to the Director as appropriate.
  • Ensure compliance with relevant legislation, professional college standards, Accreditation requirements, and organizational policies.
  • Oversee risk management processes, including critical incidents, safety planning, and crisis response.
  • Promote safe and healthy community-based work practices for staff.
  • Maintain a visible leadership presence across both community and clinical settings.

System Integration & Partnership Support

  • Ensure alignment and integration of IPCT services with internal programs and organizational priorities.
  • Collaborate with the Director to operationalize partnerships with primary care providers, hospitals, mental health and addiction services, housing providers, and community agencies.
  • Participate in system tables, committees, and Communities of Practice as delegated.
  • Support the development and implementation of coordinated care pathways that improve access, continuity, and attachment to primary care.
  • Ensure alignment with Ontario Health Team priorities, integrated care models, and system performance expectations.

Equity, Inclusion & Staff Wellness

  • Apply principles of equity, intersectionality, and anti-oppressive practice in leadership and decision-making.
  • Promote culturally responsive care and inclusive team practices.
  • Support staff wellness through reflective supervision, psychological safety, and sustainable workload practices.

Knowledge and skills necessary to be successful in this role

  • Post-secondary education and/or a Master’s degree in Public Health, Quality Improvement, or a related field, and/or an equivalent combination of education and experience
  • Minimum 5 years of progressive leadership or management experience in community health, primary care, or social services.
  • Regulated Health Professional in good standing with a relevant college (e.g., CNO, OCSWSSW) is an asset.
  • Demonstrated knowledge, sensitivity, and experience working with individuals facing complex barriers to health access, including homelessness, substance use, and mental health concerns.
  • Proven experience leading quality improvement initiatives and using data to drive service improvement.
  • Lean Six Sigma is an asset
  • Experience leading interdisciplinary teams in complex, community-based environments.
  • Strong knowledge of harm reduction, trauma-informed care, Housing First, and peer-informed practice.
  • Effective critical thinking, decision making and problem-solving skills within complex and continually changing environments.
  • Knowledge of PHIPA, clinical risk management, and regulatory compliance requirements.
  • Experience with Accreditation is an asset.
  • Strong communication, coaching, and relationship-building skills.
  • Experience with Electronic Health Records (EHR) and data-informed decision-making.
  • Valid Ontario driver’s license and access to a vehicle.

Working conditions
