In this impactful role, you’ll function as a key practitioner for the Haliburton-Kawartha Peterborough Regional Assertive Community Treatment Team. You’ll perform crucial mental health assessments, develop individualized treatment plans, and offer psychosocial rehabilitation. Collaborating with a diverse multidisciplinary team, your efforts will directly enhance client care and community reintegration.
Key Responsibilities:
• Conduct thorough mental health assessments
• Collaborate on treatment and rehabilitation plans
• Deliver clinical case management services
• Provide psychosocial interventions and environmental adaptations
• Respond to crisis situations and manage client emergencies
Requirements:
• Valid certification from the College of Nurses of Ontario
• 3-5 years of experience in community mental health
• Knowledgeable in Mental Health Act and related legislation
• Strong programming skills in individual and group settings
• Ability to engage with clients from varied backgrounds
